The FormMail section of your account control panel (http://www.yourdomain.com/menu/) has detailed instructions on how to use Formmail.

A basic knowledge of HTML forms coding is helpful when building the form into your webpage. Webpage creation software that has support for adding "hidden" "input" HTML tags to the webpage's HTML code would be helpful as well.
